JOB TITLE: Coordinated Family Support (CFS) Professional
JOB OBJECTIVE & DESCRIPTION
Altruistic Leadership receives referrals from South Central Los Angeles Regional Center to assist adults 18 and over who are diagnosed with an Intellectual/Developmental Ability. We offer personalized training and support to individuals and families living within SCLARC's catchment areas and cities, including Bell, Bell Gardens, Compton, Cudahy, Downey, Huntington Park, Lynwood, Maywood, Vernon, South Gate, North Carson, Gardena, Paramount, as well as South Central Los Angeles, Watts, Leimert Park, Florence/Firestone, and West Adams communities. Our services are provided either at the home or in the community based on the specific needs of each household. Our CFS professionals provide transportation to community agencies, establishments, and programs to achieve the service goals identified in the individual's Action Plan. The CFS professional will be knowledgeable in accessing and understanding community resources to bridge service gaps in the home. Other supports include but are not limited to linkage, advocacy, and service coordination, with education and training intended to enhance family dynamics. We are seeking DEDICATED CFS professionals with experience in case management, person-centered and family-centered service delivery, and experience working with adults with Intellectual/Developmental abilities. QUALIFICATIONS:
· A minimum of Associates-level degree in a human services field of study 3 years of experience in the developmental disabilities service system.
· Be at least 18 years of age at the time of application.
· Valid CA Driver’s License
· Personal Vehicle with valid registration and insurance.
· Driving record clearance (in good standing).
· Department of Justice background clearance.
· Current TB clearance.
· Covid-19 Vaccinations + Booster.
· Valid First Aid & CPR certification.
· Bilingual is a plus - Spanish
· Ability to lift a minimum of 25 pounds.

JOB DUTIES AND RESPONSIBILITIES:
· Close service gaps for adult individuals with Intellectual/Developmental disabilities and families who live in the same household for improved family dynamics and home functionality.
· Educate, access, and support families seeking Social Security benefits and conservatorship of the serviced individual, including appeal processes.
· Provide transportation to community agencies and programs to achieve goals and objectives identified in the CFS Action Plan.
· Provide training and education to the family on the individual's diagnosis to promote healthier relationships and understanding in the home.
· Prepare teaching material for functional skills training and support for the individual and family, as needed.
· Provide training in household management, meal planning and preparation, budget and finances, relationships, career development and preparation, self-care, and advocacy.
· Provide and translate information in the family’s preferred language and level of understanding.
· Request provider material in the family’s preferred language.
· Use evidence-based approaches with cultural sensitivity to improve service outcomes.
· Identify community resources, linkages, and programs available to the family.
· Coordinate and follow up with service providers to ensure complete linkage to benefits, programs, and supports.
· Provide education and training in self-care techniques and strategies to improve the general wellness of each household member.
· Provide linkage to local support groups available to the individual and family.
· Provide future planning education and training that empowers the family unit through proper preparation and planning for the individual with I/DD.
· Provide services in compliance with HIPAA requirements.
· Follow and understand reporting procedures and protocols as a mandated reporter.
· Monitor and modify training to meet the individual’s learning style.
· Provide training that is flexible and adaptable to the individual needs of each person.
· Complete CFS notes for each home visit and identify target needs, progress, barriers, and outcomes.
· Attend required company training and meetings.
· Adhere to all Altruistic Leadership policies, procedures, and mission.
· Other duties and supports as assigned.
Job Type: Full-time
Pay: $21.00 - $24.00 per hour
Expected hours: 40 – 50 per week
