Mobile Clinical Care, includes in home visits, visits in the mobile medical unit and telemedicine visits during travel to patient homes – 30 Hours per Week

Driver of Medical Van will be either a Community Health Worker or Expanded Practice Dental Hygienist for Mobile Medical Routes.

Clinical Duties from Florence Clinic 10 hours per week

OREGON INTEGRATED HEALTH

Oregon Integrated Health is an Integrated Primary Care Medical Home that is hiring for a Full Time Primary Care Provider for our mobile medicine unit to reach home bound patients. One day a week clinical care at our Florence office.

Our Integrated Medical Home in Florence currently has a team of Family Nurse Practitioner, Licensed Professional Counselor, Medical Assistants, a Community Health Worker, Front Desk and our Senior Director of Clinical Operations. Our patients have access to Quest Laboratories directly in our facility. We are also expanding our space to initiate the Vaccine for Children program.

Reaching the Homebound Patient Florence Coastal Oregon, Lane County is a designated Health Professional Shortage Area (HPSA). This geographic region is populated with approximately 40% persons over the age of 65 and more than 17% of the residents living in poverty. From the US Census Bureau, the major population belongs to rural areas. Even with the few medical clinics and hospitals, the number of patients that are underserved overwhelm the current structure. A mobile medical van that incorporates telemedicine as well as medical transportation will play a vital role in connecting patients to clinicians at a lower cost and with a wider reach.

Primary care services would involve a clinical consultation with a patient using live interactive video, in home visits or visits in the mobile van – depending on the patient and the appointment needed. Mobile Medical Vans that provide home assessments, access to a community health workers and Telemedicine are particularly helpful in rural areas, where the shortage of healthcare providers has led to a lack of accessibility to basic healthcare. Oregon Integrated Health (OIH), established in 2011 in Portland and expanded in 2015 in Eugene with the Expansion of Primary Care Grant from Trillium and later in 2017 in Florence, 2021 in Salem. Our business has developed a foundation of workflows, procedures and policies to successfully meet the needs of Medicaid and Medicare members. Our intention and commitment is to grow in this rural community and expand the delivery of primary care through the use of mobile and telemedicine services.

This position qualifies for the Oregon Loan Repayment Program

JOB DETAILS

· $3000 Sign On Bonus, paid first pay period following 90 days of employment with OIH

· 401K Benefit: OIH matches 100% of your contributions up to 3% of your compensation, plus OIH matches 50% on your deferrals that are greater than 3%, but less than or equal to 5% of your compensation (Eligibility Requirements: 1yr employment, 1000hrs worked, at least 21yrs of age)

· Eligible for OIH Benefits after 90 Day Probation Period

· State Medical License and DEA 100% reimbursed, completing 90 days of employment

· Continuing Education Benefit up to $700 annually, completing 90 days of employment

· 8 Holidays Per Year:

o 7 Clinic Closures, 1 Floating Holiday, eligible immediately upon hire

· PTO 160 Hours Per Year:

o Vacation PTO accrues based on time worked, with a maximum annual accrual of 80 hours per FTE 1.0, available for use after 90 days of employment

o Frontloaded PTO is uploaded based on time worked, with a maximum annual eligibility of 80 hours per FTE 1.0, available for use after 90 days of employment

o Protected Sick Time accrues 1 hour for every 30 hours worked, max use of 40 per year, available immediately as time accrues after first day of employment

JOB DUTIES AND RESPONSIBILITIES:

· The Primary Care Provider will provide primary care on a mobile medical van as well as a clinical setting with various other specialties at the clinic.

· Primary Care Provider will have access to Medical Assistants that when on the mobile unit will be able to submit referrals and perform electronic orders as submitted by provider virtually.

· Primary Care Provider will have access to Community Health Workers that will be a resource for patients in need of community resources, flex funds, in home assessments as well as follow up on ED visits and support complex care patients with primary care.

· New patient visits are 45 minutes and follow up 30 minutes, Urgent appointments are 15 minutes. Same day appointments will be available for each provider.

· Conduct standard medical and physical examinations and perform standard diagnostic tests in office and through telemedicine conferencing.

· Orders, reviews and provides patient instructions regarding diet and activities and their modifications as appropriate.

· Refer patients to other health professionals in the treatment of diseases requiring further diagnosis and evaluation.

· Provides appropriate medical record documentation in an EHR on all patients in accordance with clinic policy.

QUALIFICATIONS, SKILLS AND ABILITIES

· Previous experience in an Urgent or Family Care, Emergency preferred

· Comfortable and skilled with electronic platforms such as EHR

· Accredited Medical School Graduate.

· State of Oregon license

· Full prescription authority

· Drug Enforcement Agency Certificate

· National Provider Identifier (NPI)
