JOB DESCRIPTION:
The individual in this position provides nursing care to assigned patients under the supervision of the registered nurse. The LVN works cooperatively with ancillary, nursing and other health care team members, including the patient and his/her family, in maintaining quality care.
JOB DUTIES:
- Using data to do analytical tasks and charting tasks related to patient information
- Communicating with patients about their status and condition through emails, phone and face-to-face conversations
- Managing patient accounts with contact information, insurance and financial information in a confidential manner
- Finding appropriate clinical care teams best suited to each patient's needs
- Coordinating care with a patient's clinical team
- Assessing patients for necessary care or changes to current care plans
POSITION QUALIFICATIONS:
· Must be a high school graduate or have a GED
· A minimum of one year full time experience preferred, in the last three years in direct patient care, institutional setting (hospital or nursing facility) or agency setting
· Satisfactorily completed a competency evaluation program which complies with State and Federal guidelines
· Must have valid State of Texas LVN license
· Must have reliable Transportation
· Must have current CPR Card
· Must have current TB or Chest X-ray
· Must have Social Security Card or Birth Certificate
· Must have authorization to work in the USA, if applicable

Pay: ($28.00 per visit rate (taxable) + $12.00 mileage (nontaxable), $40.00 total per visit)
