POSITION SUMMARY: Responsible for the direct and indirect nursing care of patients in the home under the supervision of an RN. In collaboration with the patient, family and health care team, the LPN will implement and evaluate with the RN the nursing care according to the following systems/functions
Minimum Qualifications:
-Graduate of an LPN school
-Current licensure in the State of West Virginia or appropriate state where services will be provided to practice licensed practical nursing.
-Must possess valid WV or neighboring state driver’s license and must maintain WV or neighboring state minimum auto insurance
-Current CPR card
Job Types: Per diem, Part-time, PRN, Full-time
