Educate and counsel patients using sound nutrition practices for people with chronic health conditions, including complex cardiometabolic conditions. Use current nutrition research and solid counseling skills to promote health, prevent or treat illness in both individual and group settings.
Supports physicians and other health care providers to coordinate both medical and nutrition needs of patients.
Work Activities and Responsibilities:
- Screen, assess, develop and implement nutrition care plans based on patient nutritional and educational needs, following departmental guidelines for quality nutrition care.
- Use sound counseling techniques such as motivational interviewing, skills-based counseling.
- Adhere to evidence-based standards
- Maintain consistent messages with other RDNs at SDC.
- Communicate with healthcare providers both within SDC and outside of the organization to coordinate best patient care.
- Back up other RDNs to provide coverage as needed
- Teach basic cooking classes as needed
- Develop power point presentations and present classes related to diet and health.
- Give educational presentations in group settings for community forums and other groups as requested.
- Provide consistent nutrition information based on department standards.
- Chart appropriately, consistently and timely for all patient sessions.
- Stay current with research in the field of nutrition and health.
- Support organization philanthropic interests
- Perform other duties as assigned
- Report directly to Lead Dietitian
- Assist with onboarding and educating RDNs, Wellness & Cardiac Rehab staff on nutritional knowledge & guidelines
Training and Requirements:
- Bachelor’s degree in Nutrition Sciences. Master’s degree preferred.
- RDN registration in accordance with the Commission on Dietetic Registration, required.
- Advanced Practitioner Certification and other advance practice preferred.
- Minimum of three to five years of professional, clinical experience as an RDN; cardiometabolic knowledge and outpatient MNT experience required.
- Experience with computers and programs including Microsoft Office required, experience in EPIC preferred.
- Current BLS required.
