We are searching for a Registered Nurse – Supportive Care Case Manager to perform a variety of duties within agency and department policies and procedures, assume responsibility for the primary care of assigned Hospice and Palliative Care patients and their families/caregivers. Assess, develop, implement, and evaluate an individualized plan of care to maximize the comfort and health of the Hospice and Palliative Care patient and family/caregiver. Responsible for identifying patient/family/caregiver needs and for providing supportive care in accordance with the attending Physician’s orders, Plan of Care and Hospice and Home Care policies and procedures.
Hours: 40 hours (full time)
Shift: Day Shift - 8am-4:30pm - w/weekend & holiday rotations
Travel: Will require driving throughout the Southcoast Region, including Rhode Island (must use own vehicle; mileage reimbursement offered)
Education and/or Experience Requirements:
- Graduate of an NLN accredited school of Nursing is required.
- Bachelor of Science in Nursing is preferred.
- MA and RI License required. (RI license may be obtained within 3 months of hire)
- Two years of recent hospital, medical/surgical or other relevant nursing experience is required.
- Hospice & Palliative Care Certification required upon eligibility or within 2 years of hire.
- Demonstrated knowledge of I.V. pump, and central line maintenance preferred.
- Possesses knowledge and skills necessary to provide care, which is appropriate to the population of patients served.
- Proficient computer skills, including experience with various software, database and electronic records programs is preferred
