Do you enjoy helping support older adults to develop their own plan for aging well? The Northeast Kingdom Council on Aging has an opening for a Case Manager in our St. Johnsbury office. As a Case Manager, you will be responsible for assessing, development of care planning and supporting older Vermonters who wish to remain in their homes. This position requires traveling in Caledonia County.
Ideal applicants have strong communications skills and a willingness to explore options for resourceful planning that helps clients live in the setting of their choice. We are looking for someone who has a passion for helping older adults and adults living with disabilities realize their goals for independence. The ability to work cooperatively with community partners to ensure an integrated approach is achieved is essential. Home visiting is required.
Skills to be successful in this job include exceptional communication skills, both written and verbal, as well as use of computer and database system. Accuracy, attention to detail and the ability to maintain confidentiality are essential skills.
