Job Description - Facilitator (Harbor Supported Living Services)
We are seeking a compassionate, self-motivated, and energetic individual to join our team as a Facilitator in an administrative, supervisory role. As a Facilitator, you will oversee and support independently functioning teams of direct support staff to provide the best quality of support to individuals with developmental and intellectual disabilities.

Harbor Supported Living Services endeavors to assist individuals living with developmental, physical, and intellectual disabilities to live in the least restrictive, most autonomous, enriching, and self-directed environment possible. Accordingly, we assist and support individuals to choose and live in their own homes, have profitable jobs, develop friendships, and participate in community life activities.
Responsibilities:
- Coordinate services for assigned individuals, including developing and maintaining Individualized Support Plans (ISP's) and assisting with Individual Program Plan (IPP) meetings.
- Maintain individual records and provide case management, including progress reports and emergency information.
- Assist individuals in accessing generic resources and act as a referral source for community resources.
- Lead team meetings, provide supervision to staff, and manage support staff schedules.
- Participate in recruitment, hiring, training, and evaluations of direct support professionals.
- Serve as a liaison to San Andreas Regional Center Service Coordinators.
- Review staff performance and conduct staff evaluations.
- Assist with presentations to funding resources and other agencies.
- Observe and evaluate staff during work performance, both in-home and in the community.
- Attend weekly meetings with the management team and provide assistance to other facilitators when needed.
- Complete various administrative tasks, such as monthly paperwork, payroll review, and updating scheduling databases.
Knowledge, Abilities, and Skills:
- Strong understanding and knowledge of the needs of persons with disabilities.
- Effective communication skills (verbal and written).
- Crisis management capabilities.
- Familiarity with available resources for persons with disabilities.
- Ability to work effectively with program staff, community members, and families.
- Leadership skills, conflict resolution, and mediation abilities.
- Highly organized and able to manage multiple tasks with scrupulous time management.
Education/Experience:
- Bachelor's degree and/or five years of experience providing services to persons with disabilities, or equivalent experience.
- At least two years of experience in a supported living, independent living, or other person centered service environment(s)
- At least two years of supervisory experience.
- Completion of Competency-Based Training classes provided by Harbor Supported Living Services within the first 90 days of employment.
- Valid California driver's license with proper insurance.
Salary is Negotiable based on education background, experience, and successful interview
Clarification regarding scheduling:
The Facilitator position is a full time, exempt position with some on-call responsibilities that will be further discussed if chosen to interview.
