PACE (P*rogram of All-Inclusive Care for the E*lderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.
Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home.a Participants receive in home care services and transportation to a center for primary medical care, social and recreational activities, and other senior care services.

Behavioral Health Clinician Position Summary: Under the supervision of the Director of Mental and Behavioral Health (MBH) this position participates in a multi-disciplinary team providing for evaluation, individual, family and group treatment, and case management of older adults who have mental health and behavioral management needs. This position may also provide direct support for MBH program materials development and program implementation
Behavioral Health Clinician Duties and Responsibilities:
- Provides crisis intervention and management to prevent hospitalization.
- Performs intakes and assesses participants with psychiatric, psychological and/or behavioral management needs.
- Provides individual and/or family therapy for participants, their family or caregivers.
- Provides case management services as needed, including intensive case management, to include evaluations and service plan development.
- Coordinates with members of the Interdisciplinary Team.
- Leads therapy groups, for the treatment and monitoring of participants able to benefit from group therapy.
- Contributes to team meetings and case conferences through presentation of cases, discussion of clinical issues and exchange of information and ideas.
- Provides information regarding community resources to the general public as needed.
- Able to provide direct supervision to LCSW or MFT license eligible staff.
- Assist psychologist in developing and implementation of programs (e.g. behavior management program) and policies in support of the Mental and Behavioral Health Team
- Assist psychologist in providing supervision to mental health team staff and ensuring quality control for mental health services.
- Assist psychologist in assessing staff training requirements and create appropriate training programs and materials in the area of mental health.
- May help plan, develop, and administer staff training seminars and materials

Behavioral Health Clinician Qualifications (knowledge, skills, abilities):
- Master's degree in Social Work, Counseling, or Psychology
- 1 year experience with geriatric population or in long-term care setting.
- Comprehensive range of mental health and substance abuse counseling, assessment and crisis intervention skills.
- Excellent verbal and written communication skills.
- Good interpersonal skills, including ability to work cooperatively on an interdisciplinary team, and to work with clients who manifest complex psychosocial issues.
- Ability to work with various ethnic backgrounds and cultural orientations.
- Bilingual ability may be required for certain positions/assignments.
Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
