We are looking for a dynamic Baylor Registered Nurse (RN) to join our phenomenal team of clinicians!
The Baylor RN will complete full assessments, initial evaluations, re-evaluations and re-visits for patients that require treatment Friday, Saturday, Sunday and Monday. The Baylor RN is not assigned to a designated area but covers the service area for a Branch. This is performed in accordance with physician orders and plan of care under the guidance and supervision of the Director of Clinical Services/Nursing Supervisor.
RESPONSIBILIITES:
- Follow a designated care plan tailored to each patient specific needs.
- Makes the initial evaluation visit and regulary re-evaluates the patient nursing needs.
- Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
- Informs clinical team of changes in the condition and needs of the patient.
- Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those patients requiring their specialized skills.
- Participates in and presents in-service programs.
QUALIFICATIONS:
- Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
- Active license in the state(s) to which you practice.
- Minimum of 1 year nursing experience in a med-surg, ICU, emergency room environment, preferred.
- Minimum of 2 years Home Health experience, strongly preferred.
- Minimum of 1 year experience using Point Care (HCHB) EMR system, strongly preferred.
- Acceptance of philosophy and goals of this Agency.
- Ability to exercise initiative and independent judgment.
