Southern Tier AIDS Program, Inc. is seeking a Prevention Care Navigator for our Mobile programs. This position requires employees to work on our mobile van and applicants must have a level of comfort working out in the community, working out in the elements, and in close proximity to other staff and clients. This position requires a valid driver’s license and clean driving record and applicants must be willing to drive a 15-passenger van. The starting salary for this position includes a per hour bump as this position is budgeted for less than two years.
This position provides linkage and navigation services to participants within STAP/STCC harm reduction programs. Provides referrals, harm reduction education, and outcome follow-up for participating clients. Coordinates care and transportation to increase access to medical and behavioral health care, harm reduction education, testing services, as well as social service benefits & financial entitlements. Responsible for providing services to participants in outlying areas including in-person through the agency mobile syringe exchange unit and through available phone and telemed options. Offers low threshold care coordination services, as well as completing intake, risk assessment, and HIV/HCV testing services.
Salary:
- $21.00/hour working 35-hours per week, may vary depending upon experience.

- High School Diploma and a minimum two years of relevant experience in a human service or community based organization; OR associate’s degree and one year relevant experience in human services or community based organization.
- Lived/personal experience can substitute for a year of experience.
- Minimum of one year working in human services required.
- Experience in a directly related field (case management, syringe exchange, HIV/HCV
services, or substance use treatment) strongly preferred.
- Previous experience working with individuals actively using substances is strongly
desired.
- Experience serving individuals with a Substance Use Disorder (SUD) or Opiate Use Disorder (OUD) diagnosis is preferred.
- Computer literacy, organizational skills, public speaking and presentation skills, strong interpersonal skills, knowledge and acceptance of the Harm Reduction philosophy required.
- Excellent organizational, strong time management and communication (verbal
and written) skills are needed.
- Ability to work autonomously with good judgment and record keeping skills are essential.
- Computer literacy required.
