JOB DESCRIPTION
JOB TITLE: Care Manager
REPORTS TO: Care Manager Lead
Responsible for opening the consumer case which includes completing the assessment and documenting the same, completing orientation of the direct care workers both in the office and in the home setting to the consumer’s care/case. Supervises the direct care workers onsite per regulatory guidelines and completes wellness checks on existing consumers. Documents all incident reports per regulatory guidelines in the EIM system and documents all consumer/ customer concerns.

Opens consumer cases and completes the assessment which includes
-
Assesses the consumer needs and completes the non-medical assessment in its entirety.

Reviews consumer booklet with the consumer and/or significant others and completes all required forms per regulatory guidelines, confirming they are signed and dated.

Develops or reviews the consumer’s care plan in relation to the consumer’s needs.

Orients direct care workers to their role and responsibilities and completes their:

Orientation checklist
-
Orientation to the individual consumer case and documents in HHAeXchange so that the worker understands the care plan and can appropriately document assigned tasks and clock in and out in EVV system correctly.

Completes wellness checks and documents the same, reflecting any necessary updates or changes to the consumer’s care

Maintains productivity per agency standards which is targeted to be:

30 consumer assessments per week
-
30 wellness checks per week
-
Supervises the direct care workers related to consumers on caseload and documents the same in their file using the Supervision Form.

Documents supervision of any provisionally hired direct care worker awaiting their background check.

Documentation is completed for 30 days for a Pennsylvania resident and 90 days for an out-of-state resident.

Documentation reflects the monitoring results for the direct care worker completed through random direct observation and consumer feedback that is written in order to be placed in the direct care worker’s personnel file by Human Resources.

Completes any consumer incident reporting within 24 hours of knowledge through agency incident reporting or EIM system documenting events per regulatory guidelines. Seeks guidance from Care Manager Lead or Director of Branch Operations if uncertain as to where incident should be reported.

Documents consumer concerns and complaints on agency Customer Concern and Complaint form. Seeks guidance from Care Manager Lead or Director of Branch Operations if uncertain as to how to document complaint.

Must maintain a professional attitude and demeanor and be able to communicate effectively and relate courteously and cooperatively with consumers, caregivers, colleagues, supervisor,
co-workers and all others. -
Must be able to demonstrate optimism, enthusiasm and willingness to work constructively with other home care team members and reflect concern for the well-being of consumers and others.

Able to operate basic office equipment (copier, fax, computer, telephone)

Ability to frequently stand, sit, bend, walk and drive without reasonable accommodations to perform the essential responsibilities of the position.

Driving and local travel is required in this position.

Functions in a variety of settings from the office as well as in consumer homes and may be subjected to a wide variety of environments and temperatures as a result.

High School Diploma at minimum

Associates Degree or the equivalent preferred.

LPN or CNA preferred
Experience:
-
Minimum of two years’ home care management or care coordination experience preferred. (Candidates with exceptional work or personal experience in home care may be considered).

Must have excellent organizational, communication (written and oral), and interpersonal skills, including being able to demonstrate flexibility, adaptability, tolerance, patience, and tact.

The candidate must have experience of interacting effectively with people with disabilities.

Must also have the ability to maintain confidentiality, work independently and as a team player, and be detail oriented.
Other Requirements:
a. Possesses valid Pennsylvania driver’s license with reliable transportation insured in
accordance with Pennsylvania state laws and regulations.
b. Has satisfactory references from employers and/or professional peers.
c. Possesses satisfactory criminal background check.-
