Turning The Corner Residential Program is looking for a full-time Care Manager. The ideal candidate would have experience in the youth residential care field as well as office management experience. Care Managers are responsible for ensuring that all Turning The Corner residents' files are always inspection ready. They will also ensure that all facets of the resident’s treatment plan are active and will advocate for each resident as is appropriate. Care Managers must direct communications, both internal and external are completed in a timely manner, all appointments are logged and recorded for staff, residents and external providers are informed of meetings and other necessary communications. A successful candidate will have experience working with youth in residential programs, working with DCYF , Probation and other care providers. TTC is looking for a team player who can be a part of the important work we are doing in the care sector.
The job scope and function, includes but is not limited to:
- Processing all intake paper work.
- Participating in the administrative team meeting as necessary
- Ensuring and or gather from the referring agency the following documents for new residents: psychiatric evaluations, immunization record, social history, current case plan, medical records, IEP, current grades, birth certificate, social security card, prescriptions and any other appropriate documentation.
- Ensuring that all new clients who enter the program are register with the school department within 24 hours of admission or schedule GED classes if the resident has legally withdrawn from school..
- Ensuring that within 24 hours of intake Primary Care Physician is assigned to client and that the insurance company is notified of new address. Ensure that a medical, dental and eye appointment is schedule for each new client within 15 days of admission
- Ensuring residents are signed up for Life Skill Classes and Teen Challenge grants
- Ensuring clients are transported to all medical, court and educational appointments.
- Maintaining a weekly record of all client appointments and passes.
- Updating the daily census which list all Admissions, Discharges, Absent From Care and Overnight Hospital Stays, etc.
- Reviewing each resident's binder
- Performing all assigned office to include answering the telephone, clearing the voicemail, purchasing office supplies.
- Willing to provide direct care services to residents as needed.
- Performing all reasonable task as assigned by Program Director.
- The preferred candidate will have at least 2 years-experience with office management, experience using Microsoft Office tools, proficiency with data management collection, be organized and self-motivated, willing to have flexibility in their schedule, possess a valid driver’s license and be able to pass all background clearances
