Our staff of credentialed therapists offers comprehensive psychological care for a broad spectrum of mental health issues and disorders. We are looking to add psychiatric nurse practitioners to our continuum of care in order to provide wrap around services to our patients inside and outside of facilities. SUMMARY/OBJECTIVE:
The Nurse Practitioner (NP) for THS is a clinical role working directly within the clinical team and oversight is provided by our Medical Director (MD). The NP will provide direct patient care to skilled nursing facilities and rehab patients. The NP will meet with patients for initial assessments, and diagnosis, and continue to follow treatment plans as outlined. Our goal is to deliver personalized quality services to those in post-acute and long-term care. The Nurse Practitioner will serve as the front line of care and assist the clinical team of psychologists, social workers, and interns accordingly.
The role of the Nurse Practitioner will largely be part of a facility-based team of providers who deliver services to patients residing in care facilities. We strive to give quality care to residents while working together with the facility to carry out the best care possible using an interdisciplinary care team approach, allowing NPs an opportunity to positively impact the overall well-being of the elderly population.
ESSENTIAL JOB DUTIES:
- Deliver patient care based on the guidelines as issued by the State of Florida and permissible under their license identifying the scope of practice while providing primary and preventative care for long-term care patients residing in skilled nursing facilities.
- Prescribe appropriate medications, including the occasional controlled substance, in accordance with FL Law.
- Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and/or the care plan.
- Initial provider at the facilities working with THS and any new facilities in the area assigned.
- Evaluate new admissions and schedule routine follow-ups based on guidelines and be present to deliver acute care daily.
- Occasionally update family members on patient status and communication, especially where a POA is present.
- Work directly with the facility to maintain compliance with appropriate documentation, as well as maintenance of logs and notes in THS’s EMR
- Create a partnership between care coordination/oversight and care delivery at the facility.
- Work directly with operations support staff and employees designated at the facility level to identify, track, and report, on initial/continued services for seamless care.
REQUIRED EDUCATION/ EXPERIENCE:
- Current, unrestricted NP license in the state of Florida with DEA licensure
- Clinical background and/or experience working in a long-term care setting with an adult, family, or geriatric specialties.
- Ability to work within an interdisciplinary care team to coordinate patient care.
- Nurse Practitioner – Qualifications:
- Nurse Practitioner certification REQUIRED (ANCC certification or equivalent)
- Long Term Care skilled nursing facility (SNF) experience preferred.
- Must have a strong clinical background and experience in SNF, hospital, or acute care settings.
- Must have strong interpersonal skills, be a team player, and have the ability to work independently.
- Advanced medical knowledge of common chronic disease states
- Previous experience with EMR and basic computer skills required.
- Prescribing and managing medications in accordance with patient treatment plans
- Diagnosing and assessing psychiatric problems and illnesses
- 3-5 years’ experience in geriatrics preferred, but not required.
- The ideal candidate will have experience working for a mobile practice.
