The Hospice RNs at Carter Healthcare operates with a case management approach, meaning our RNs are the direct point of contact for patient care and information. The RNs develop a holistic, multidisciplinary Plan of Care, as well as:
- Accountability for the coordination of care
- Implementation of physician’s orders
- Provision of care from admission to discharge
- Provides clinical supervision to the members of the Case Management Team (I.E., CHHA, Level 1 RN, LPN, student, Nurse Preceptor, care associate.)
- Responsible for providing care and documentation in a comprehensive, competent and thorough manner

Qualifications:
- Licensed as a Registered Nurse in the State of Oklahoma; B.S. preferred
- Home care/Hospice experience preferred
- Proficient at venipuncture/phlebotomy
- Cardiopulmonary resuscitation
- Electro-cardiograms (ECGs)
- Infusion therapy including intravenous medication administration
- Post/access device care
- Basic rehabilitation therapy
- Knowledge of Medicare, private insurance and DME regulations, JCHAO, DHS, local state and federal regulations. Broad knowledge of principles of nursing and rehab home health and their application
- Previous Case Management experience preferred
- Ability to provide an organized, efficient approach to caseload management
- Ability to demonstrate understanding and support of agency policies, legal considerations and professional ethics
