**Overview:**
We are seeking a dedicated and experienced Director of Social Services to join our team. The ideal candidate will oversee and manage the social services department, ensuring the delivery of high-quality care and support to individuals with developmental disabilities.
**Responsibilities:**
-Works directly with the Administrator on matters contributing to the psychosocial well being of residents, as well as other issues.
-Insures that the Social Workers provide services such as but no limited to referral, counseling, advocacy, liaison and support to the residents and families.
-Insures that the Social Woorker is responsible for the assessment and intervention of medically related psychosocial needs of the residents.
-Ensures that the Social Work Department adheres to all applicable governmental regulations, by staying abreast of new requirements. Provides social work staff opportunites to attend ongoing education regardign DSS requirments, Medicare, and transfer/dishcarge regulations.
- Assume a caeload as assigned by the Administrator. Assess residents from time of admission to develop tratment plans, document interventions and pertinent information in the medical record.
- Develop discharge plans for residents and coordinate all discharge planning in accordance with policies and procedures.
-Keep the resident's family or significant others informed of the resident's status and refer the family to the appropriate discipline for informaiton or to resolve concerns or complaints.
- Make consistent efforts to have residents and their families/significant others attend Patient Care Concferences on a regular basis.
-Assist with information and referral regarding Medicaid application.
-Organize/lead Family Council as requrested by the Administrator
