Our campus is comprised of a Skilled Nursing Facility serving 59 residents, 26 Assisted Living apartments , and two Adult Day Care programs.
JOB TITLE: Clinical Manager MDS Coordinator
QUALIFICATIONS: License in MN as a RN
EXPOSURE RISK: Routine tasks involve exposure to blood, blood-containing fluids or tissues.

RESPONSIBILITIES AND DUTIES
- Manage own time to accomplish assigned duties.
- Present appearance consistent with the facility dress code, wear name tag.
- Come to work as scheduled and consistently demonstrate dependability and punctuality.
- Assist new employees in following established policies and procedures.
- Assure resident rights are respected in all resident situations.
- Answer any call light/need promptly and obtain appropriate assistance as
needed
- Respond to all emergency situations as needed, drills and real emergencies.
- Follow standard precautions and infection control practices.
- Follow SHCC Vulnerable Adult policy. Promote Resident Rights.
- Follow departmental Safety Rules and promote safety.
- Use and promote positive working relationships with co-workers and other departments (teamwork).
· Use and promote effective communication techniques with residents, families, visitors, co-workers and other health care professionals.
- Promote positive image of SHCC.
- Address resident/staff issues in private.
- Address resident concerns and take appropriate action to correct. Contact the correct supervisor if concerns become known.
- Attend mandatory in services to improve knowledge and skills to maintain competency.
- Attend and participate in meetings and committees, collect, record and report data as needed.
- Participate in Quality Projects as needed and asked to do.
- Develop individual comprehensive resident care plans based on subjective and objective data obtained from resident, family, and staff interviews, medical history, diagnosis and nursing assessment. Review and update quarterly and as needed.
- Complete RAI per facility policy & procedures & governmental regulations.
- Understands and completes MDS in regard to payment for State and Federal Programs.
- Plans and implements information gathering and MDS completion for residents.
- Communicates with staff, family, residents and other sources for MDS process completion.
- Monitors resident status, noting changes in need of medical, social intervention. Collects and evaluates data for decision making in MDS completion.
- Assists in coordinating nursing services with other resident services. Meets with staff to identify and correct problems.
- Preforms the pre-admission screening of potential residents.
- Monitor Medicare in the facility
- Make Medicare coverage determinations on admit to assure a favorable “waiver of liability” status for the facility. Informs the Social Worker of non-coverage for Medicare and issues “Notice of Non-coverage Under Medicare” as required. When appropriate initiates “Requests for Reconsideration” for the facility and follows required procedures related to this process.
- Oversee and assign monitoring of Tardive Dyskinesia/Psychotropic
medications and complete Discus as needed.
- Perform or coordinates monitoring of rehab program, resident care audits, and infection control program on assigned wings.
- Monitors that documentation on wing is complete, does audits of nurse and aide charting.
- Ensures that group lists and care sheets are updated weekly.
- Conducts and oversees care conferences, updating family on resident’s progress.
- Ensures that resident needs are met by ensuring care plans are current and individualized.
- Perform wound rounds weekly, monitor for skin issues, cuts, bruises
- Reports and Participation for QAPI as needed.
- Perform other duties as assigned by the Director of Nursing.
