We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. Your role will involve assessing client needs, developing care plans, coordinating services, and advocating for clients' well-being. Responsibilities:
- Conduct comprehensive assessments of clients' needs, including physical, emotional, and social aspects
- Develop individualized care plans based on assessment findings
- Coordinate and monitor services provided to clients, ensuring they align with the care plan
- Collaborate with healthcare professionals, social workers, and other stakeholders to ensure holistic care for clients
- Advocate for clients' rights and well-being
- Provide counseling and support to clients and their families
- Maintain accurate and up-to-date documentation of client interactions and progress
- Monitor client progress and make adjustments to care plans as needed
- Stay informed about community resources and refer clients to appropriate services
Experience:
- Previous experience in case management or a Hospice field is preferred
- Knowledgable about Hospice EMR system like HospiceMD
- Knowledge of laboratory systems, aseptic technique, occupational health, memory care, phlebotomy techniques, toddler care, administering injections, inhaling medications, and conducting physical examinations is beneficial
- Strong communication skills with the ability to effectively interact with clients from diverse backgrounds
- Excellent organizational skills with the ability to manage multiple cases simultaneously
- Empathy and compassion for individuals facing challenging circumstances
- Ability to work independently as well as collaboratively within a team

Requirements:
-Current LVN licensure in the state of California
-Minimum of 2 years of experience in a Hospice industry
-Bilingual (English/Spanish) preferred.
