The Case Manager will be responsible for documenting case notes and program activities, entering data, and preparing monthly reports reflecting such activities, as indicated. The Case Manager will perform direct service work in support of at-risk/foster youth who have a large potential to drop out of school. The main focus of the position is to support participants in having the necessary resources and services to complete middle and high school, explore post-secondary education, and career options, and navigate the process of acquiring gainful employment. This position also provides ongoing support, advocacy, and guidance to participants to assist them in dealing with their personal and professional habits, especially those that interfere with their ability to meet the program requirements and their personal goals. Additionally, this position will work closely with program staff to build the youth’s leadership skills, to better advocate for themselves.
What you will be doing:
- Conduct zoom or video conferencing meetings with 6 – 10 students / parents
- Conduct online zoom training, go to meetings , webinar classes with students
- Conduct pre and post assessment of the service need of students
- Ability to do Data Tracking on youth services provided
- Providing linkages and established referral systems with school and community services and resources that support the development of education, employability and college preparedness
- Monitoring services to ensure that they are received, adequate to meet the client’s needs, and consistent with quality care
- Tracking participants and keeping electronic communication while in program and with those who have exited the program
- Communicating with supervisor to assess student’s progress and needs
- Advocating and being a positive ally for all participants
- Assisting participants with obtaining vital life records (birth certificate, social security cards, etc.)
- Coordinating planned recognition or award ceremonies and events
- Assisting the instructional staff when needed
- Participating in regularly held staff meetings and case reviews
- Building and maintaining solid relationships with those students in follow-up, offering support for their continued success in seeking placements after program graduation
- Keep proper documentation of case notes and program participation
- Create monthly/quarterly and yearly reports of youth and program progress
Qualifications
- AA/AS or current student with equivalent of college credits in Social Work, Education, Psychology or closely related field
- 1+ years of experience working with at-risk youth ages 14-21
- 2+ years of experience working in educational, vocational or career development is a plus
- Ability to manage and run zoom meeting
- Must be able to work independently and manage time effectively.
- Must have a positive attitude and like to help people.
- Can manage time wisely and meet rigorous deadlines.
- Must have experience in maintaining proper documentation of case notes and program participation.
- Must have the ability in maintaining follow-up and proper documentation of case notes for students.
- Possess good communication, written, and organizational skills as well as complete paperwork in a timely manner.
