Job Summary:
The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. Provides direct care and case management for a team of Agency clients. Demonstrates proficient skills using HIS data set to admit, case manage, and discharge hospice patients in regards to physician orders. Coordinate and supervise LPN’s and HHAs in the delivery of patient care. Maintain compliance with agency policy and procedures. Follow Medicare and Medicaid regulations.
Job Functions & Responsibilities
- Reviews client referral information and collaborates with the clinical supervisor regarding eligibility and appropriateness for hospice services. Coordinates and plans interdisciplinary group additional assessments of patient and family to determine hospice care needs.
- Uses the palliative care model, seeks input from IDG/IDT, physician and the patient and family goals for disease course to determine the plan of care and establishing outcomes.
- Effectively manages patient and family palliative expectations regarding agency services and works with IDG/IDT and patient/family/ caregivers to deliver effective, efficient and desired outcomes.
- Manages proper utilization of hospice resources and implements IDG/IDT approved (ordered) interventions.
- Coordinates and delivers IDG/IDT’s approved (ordered) care plan, incorporating appropriate skilled interventions and necessary medical supplies/equipment and ancillary/specialty services, to achieve desired outcomes.
- Projects realistic visits by discipline and medical supplies required per IDT/IDTs planned interventions and desired outcomes.
- Regularly evaluates hospice patient’s progress, in collaboration with IDG/IDT; who revise patient POC accordingly.
- Administers medications and treatments as prescribed by the physician, IDG/IDT and per patient’s POC accordingly.
- Identifies desired hospice patient’s desired outcomes when developing the plan of care; identifies and implements community referral support.
- Provides for IDG/IDT and primary physician, when necessary and every two weeks.
- Participates in conferences of communications with IDT to obtain/receive treatment changes; and communicates new/changed orders to appropriate team members.
- Facilitates ongoing care discussions with IDG/IDT and team members regarding needs for ongoing care, revisions of interventions to achieve desired outcomes.
- Monitors documentation of clinical records to ensure compliance with regulatory standards of timeliness, accuracy and completeness.
- Manages/supervises a team of RNs, LPNs, and Home Health Aides to provide effective and quality hospice services.
- Participates in human resource management to achieve quality service delivery and positive employee relations.
- Promotes personal safety and a safe environment for clients and coworkers.
- Performs job duties in accordance with Agency policies, procedures, and professional and community standards.
- Participates in the development, implementation and evaluation of the Performance Improvement Program.
- Maintains confidentiality in all aspects of job
- Ensures that arrangements for equipment and other necessary items and services are available.
- Performs other job duties as assigned.
Qualifications:
- Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting.
- Current licensure in state and CPR certification. Online CPR certification not accepted.
- Associate’s or Bachelor’s degree, with one (1) year of hospice/home health care experience preferred.
- Ability to supervise and direct members of the client care team.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist and excellent organizational and time management skills.
- Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.
- Physical requirements, with or without reasonable accommodation: prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
