Job Responsibilities:
- Oversee activities and provide both in-store and online diagnostic devices and solutions, and assist with proper use of devices to all customers
- Administration of vaccines (both IM and SQ) under a physician protocol
- Draw blood per physician request or under physician protocol
- Performs gap closure activity as outlined, and escalates issues as needed
- Effectively manage a recurring caseload of patients each month
- Manage defined patient population to close gaps in case (both clinical and non-clinical)
- Evaluates customers’ needs and can advocates on patient’s behalf to connect them to resources, services, and potential solutions related to their health condition
- Identify and coordinate community resources for patients
- Perform telephonic patient outreach
- Provides patients with education on the proper use and setup of diagnostic device (ie. Blood pressure machine, blood glucose monitors, pulse oximeter, etc).
- Effectively communicates with agency staff, liaisons, other Care Facility staff, patients, and family/caregivers
- Manages time effectively to deliver accurate and appropriate patient care
- Maintains confidentiality of patient and proprietary information
- Maintains a professional image
- Participates and attends required in services, trainings and weekly meetings
- Build and maintain a working relationship with the front end and pharmacy team to enhance patient engagement and experience
- Assisting patients in aisle by providing them education on OTC and first aid products, escorting patient to pharmacy for patient specific questions
- Maintains Health Corner space to brand standards provided by Walgreens and keeps organized records in Health space in accordance with compliance policies
Basic Qualifications:
- Active LVN/LPN license by State Board of Professional Regulations (must be able to remain current with licensures, certifications and continuing education)
- At least 2 YEARS experience as an LPN/LVN providing nursing care. Self-directed with excellent time management skills and able to work independently as well as under the supervision of a registered nurse or another clinician.
- Must be willing to obtain NPI number before employment begins
- Must be willing to obtain free Yellow Fever vaccine certification from CDC website
- Must have experience with a variety of patients (including children and the elderly)
- Flexibility and the ability to work effectively within a multidisciplinary team
- Current CPR/BLS certification, up to date on Blood borne pathogen training
- Ability to communicate effectively (verbal and written) with team members, patients, caregivers, physicians and referral sources
- Willingness to work a flexible schedule, including evening and weekend hours
- Comfortable with digital technologies such as website forms, mobile applications, PC/tablet systems, in-store systems and connected healthcare devices
- Ability to work under stressful conditions
- Ability to get to the job site
***Floater Per Diem Role*** NJ Hours of Operation: 9 am-5:30pm Monday through Friday. They receive a 30 minute lunch (unpaid). this floater could be called on Monday-Sunday to pick up shifts where needed. M-F 9-5:30pm.
