Job Summary
As a Licensed Practical Nurse (LPN) with our facility, you will play a vital role in delivering comprehensive care and support to our patients with developmental disabilities. This is a rewarding opportunity to make a meaningful difference in the life of our patient.
Responsibilities:
- Provide skilled nursing care and support to our patient in accordance with established care plans and protocols.
- Administer medications and treatments as prescribed by the healthcare provider.
- Monitor and assess the patient's condition, including vital signs, respiratory status, and overall well-being.
- Assist the patient with activities of daily living, including bathing, grooming, and mobility support.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Communicate effectively with the patient, family members, and other healthcare professionals to ensure continuity of care.
- Maintain accurate and timely documentation of nursing assessments, interventions, and outcomes
Qualifications:
- Current licensure as a Licensed Practical Nurse (LPN) in the state of New Jersey.
- Minimum of 2 years of experience working as an LPN in a healthcare setting, preferably with individuals with developmental disabilities.
- Strong clinical skills and knowledge of nursing principles and practices.
- Ability to assess and respond effectively to changes in the patient's condition.
- Excellent communication and interpersonal skills.
- Compassionate and patient-centered approach to care.
- Ability to work collaboratively as part of a multidisciplinary team.
- Basic computer skills for documentation purposes.
