Responsibilities:
- Provide comprehensive primary care services to patients of all ages, including infants, children, adolescents, adults, and seniors
- Diagnose and treat a wide range of acute and chronic illnesses
- Perform routine physical examinations and preventive screenings
- Develop and implement treatment plans for patients with chronic conditions
- Coordinate and manage patient care, including referrals to specialists and follow-up care
- Conduct discharge planning for patients transitioning from hospital to home or other care facilities
- Provide health coaching and education to patients on disease prevention, lifestyle modifications, and self-care management
- Perform minor procedures such as phlebotomy and wound care
- Collaborate with other healthcare professionals to ensure high-quality patient care
