We are seeking qualified Registered Nurses (RN) to join our Assertive Community Treatment (ACT) in Wilmington, Delaware.

The Clinician Registered Nurse as part of the ACT multidisciplinary team provides the coordination of behavioral and physical health care needs of the program participants, monitors participants' medication and provides psychiatric nursing services consistent with physician’s orders. Some duties include:
- Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participants and assigned staff.
- Maintains a case load of ACT participants and completes assigned paperwork in a timely manner.
- Act as a liaison consulting with community agencies and families to maintain coordination of care in the treatment process.
- Provides ongoing assessment of participant’s mental health symptoms and responds to recovery plans ensuring appropriate and early intervention.
Key Qualifications:
- Board Certified Licensed RN in the state of Delaware; or participating in the Nursing Licensure Compact.
- Knowledge of mental health and substance abuse systems and supports.
- Must possess a valid driver’s license with an acceptable driving record.
- Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illness and respect for participants’ rights and personal preference in treatment are essential.
Scope:
- Frequent talking and listening.
- Requires the ability to lift, carry, hold, kneel, reach, stack, stoop, and bend, which may include direct intervention with participants.
- Local travel required, including transporting program participants.
- May be required to report to work during emergencies, including inclement weather.
- Scheduling flexibility and possible on-call work are necessary.
- May require crisis intervention in emergency situations.
