Area of Responsibilities
- Accepts responsibility for coordinating physical care of the patient by teaching caregivers, volunteers, and employees
- Assumes primary responsibility for a patient/family caseload that includes the assessing, planning, implementing, and evaluating phase of the nursing process
- Initiates communication with the attending physician, Hospice staff, other agencies, and Medical Director to coordinate optimal care and use of resources for the patient and family
- Maintains up to date patient records so that problems, plans and actions and goals are accurate and clearly stated with changes reflected as they occur.
- Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care
- Assumes role of associate nurse to the primary on-call nurse for information relating to patient caseload
- Demonstrates familiarity with policies and procedures of the Agency and understanding of State and Federal regulations
- Interviews and/orients potential and new employees as required
- Maintains accurate inventory of supplies delivered to patients
- Maintains regular communication with the Clinical Manager to review caseload
- Meets regularly with Hospice staff to review problems or changes with caseload
- Participates in agency and community programs as requested to promote professional growth and understanding of Hospice Care
- Promote teamwork and empower teams and individuals to meet their potential
- Provides appropriate in Agency orientation and in-service education programs for staff
- Seeks input from other Hospice team members regarding the patient plan of care to obtain additional knowledge and support
- Shares in providing 24 hour, 7 day per week on-call coverage to patients
- Supervises and documents the Home Health Aide plan of care every 14 days
- Support quality and efficiency improvement efforts
Compliance Statement
Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.
