Job Summary
JMK Community Health Foundation is currently looking for Per Diem and Part Time Nurse Practitioner to assist in a Family/Internal Medicine Practice located in Gardena and Compton, Ca.
The Nurse Practitioner will be responsible for providing health care management, disease prevention and health promotion for his/her patient base. Candidates should be experienced in all aspects of Family/Adult medicine, Including family planning, birth control counseling and chronic Disease Management.
Must be able to Practice in a multicultural diverse, low-income setting.
The NP participates in continuous quality review, including systematic review of medical and other records and treatment plans on a periodic basis, maintains current knowledge by attending continuing education programs. The NP must maintain a current Furnishing Number and a DEA as well as NP Certification.
Responsibilities and Duties
- Coordinates patient care
- Have the ability to manage patients with multiple Chronic diseases and illnesses;
- Ensures following of all medical protocols/Standardized Procedures
- Participates in multidisciplinary teams to improve patient care processes and outcomes.
- Tracks quality assurance data and monitors patient care management.
- Assesses patient health by interviewing patients; performing physical examinations; obtaining, updating, and studying medical histories.
- Determines abnormal conditions by administering or ordering appropriate diagnostic tests, such as x-rays, electrocardiograms, and laboratory studies; interpreting test results.
- Documents patient care services by charting in patient and department records. (EHR documentation)
- Performs therapeutic procedures by administering injections and immunizations
- Understand when to manage conditions and referral for Co-management;
- Performs suturing; managing wounds and infections.
- Instructs and counsels patients by describing therapeutic regimens;
- Educate the patients on appropriate chronic disease management which includes normal growth and development information;
- Educate on family planning; providing counseling on emotional problems of daily living;
- Promoting wellness and health maintenance.
Qualifications and Skills
- Current Advanced Practice Registered Nurse (APRN) Licensure in CA
- Electronic Medical/Health Record (EMR) experience essential
- Excellent customer service skills
- Ability to work in a fast paced environment
- Spanish proficiency preferred but not required

Pay: $119,012.61 - $143,327.02 per year
