- Extracting relevant information from patient records.
- Liaising with physicians to clarify information on any documentation that is insufficient or unclear
- Communicate with other clinical staff regarding documentation
- Examining documents for missing information.
- Assign codes to diagnoses and procedures, using ICD (International Classification of Diseases) and CPT (Current Procedural Terminology) codes.
- Ensuring documents are grammatically correct and free from typing errors.
- Performing chart audits.
- Ensuring compliance with medical coding policies and guidelines.
- Any other tasks as assigned by supervisor
- Collaborate effectively with other members of team as appropriate
- Act as a patient advocate and maintains privacy and confidentiality of all appropriate information
