Duties:
- Coach/ assist people supported in ADL skill building including personal hygiene, bathing, meal preparation, dressing, toileting, shaving, etc., using instructional methods according to people’s individual needs and goals.
- Coach/assist people supported in developing appropriate interpersonal interactions and social situations in order to obtain normalized individual development and normalized socialization.
- Implement activities, situational experiences or leisure time for people supported using necessary equipment, vehicles, events or sites in accordance with daily routine, written schedule or ISP goals, according to the person’s interest.
- Accompany people supported on medical appointments, community outings, or events away from home using necessary personal vehicle (with mileage reimbursement), company vehicles, or public transportation in accordance with each person’s needs, goals, and planned events or in emergencies following instructions from Nurse or Supervisory personnel.
- Discuss the response/ reaction of the person supported to ISP implementation, significant events or participation in everyday activities with House Manager, Supported Living Manager and other supervisory staff in accordance with agency requirements and state or federal guidelines.
- Engage with the team of the person supported, including relatives, service coordinators, day program staff, treatment professionals and other members of the person’s support team to ensure the health, safety, and welfare of the person supported.
- Provide Behavioral and Medical Interventions as described in behavioral support plans, nursing plans, nutrition plans and other treatment related documentation.
- Support and encourage everyday use of learned skills and abilities as part of the natural daily routine in order to maximize normalized development and independent functioning.
- Perform health related tasks including giving prescribed medications (as a certified TME), take blood pressure/temperature, check skin condition, colostomy care assistance, positioning, using prescribed procedures or following QIDP/Nurse instructions in order to maintain each person’s health, safety and welfare.
- Participate in all required training including Phase I, Phase II, Phase III and Phase IV. Maintain state and federal certifications including First Aid, CPR, TME, BPS/CPI.
- Maintain knowledge of treatment plans for the person supported. Exchange and receive information about the person in verbal or written format as required in order to maintain records, treatment plans or implement services including the development of daily shift notes, logging behavioral and goal data.
- Participate in various regularly scheduled duties including household chores, emergency drills, meal preparation, grocery shopping etc., in accordance with assigned schedules and regulations in order to ensure the health, safety and welfare of the people supported.
- Substitute for absent or unavailable co-workers in performance of assigned duties with direction from supervisor in order to ensure the day-to-day functioning of the program.
- Perform miscellaneous duties as assigned by supervisor in order to ensure day-to-day functioning of the program.
