Schedule: Part Time, Days, Benefits-Eligible - Mon-Fri, 8:30am-3pm
The Nurse Care Coordinator serves in an expanded nursing role to collaborate with patients and their primary care providers to offer a model of care that ensures the delivery of quality, efficient and cost-effective healthcare services. The Nurse Care Coordinator:
- Assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status.
- Integrates evidence-based clinical guidelines, preventative guidelines, protocols and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for the populations.
- Develops systems of care that monitor patient progress and promote early intervention in acute care situations.
- Works effectively with other members of the healthcare team to optimize interventions.
- Manages utilization and practice metrics to further refine the delivery of care model to maximize clinical, quality and fiscal outcomes for identified populations.
EDUCATION AND EXPERIENCE:
- Associate's degree, diploma or certification in nursing and over 6 months to 1 year of office RN experience required.
- BSN preferred.
LICENSES AND CERTIFICATIONS:
- Basic Life Support (BLS) upon hire required.
- Registered Nurse in PA – licensed, upon hire, required.
