Individual Support Planning:

Knows the content of each ISP and partners with persons supported to follow through on goals.

discover new areas of interest.

Assists individuals to develop a positive self-image; encourage independence with adult daily living

skills providing assistance whenever needed.
Documentation and Follow-Through:

Reads the house communication log at the beginning of each shift.

Writes a clear, brief summary of the shift prior to the end of each shift.

Completes all required daily documentation as related to the ISP.
-
Is accountable for and properly documents the use of money.
Education:
-
High school Diploma or GED.
License:
-
Valid driver’s license with acceptable driving record.
Qualifications:
-
Must be at least 18 years of age.
-
Access to and provision of an insured and inspected automobile on an ongoing basis.

Ability to complete CPR, NCI, Human Rights, Management and other required trainings

Capable of effective oral and written communication
-
Capable of reading and comprehending policies and instruction, and use varying degrees of
-
independent judgment and discretion.
-
Ability to respond to emergency situations at all times.
