Hospice/Palliative Care Social Worker, MSW, Full-time

Salary: $58,000.00 - $65,000.00 per year based on experience and licensure
Coverage Area includes: Lake, Porter, La Porte, Jasper, Newton, Starke Counties. Mileage paid at the current IRS rate, if a fleet car is not available.
SUMMARY:
The Hospice/Palliative Social Worker works in conjunction with the interdisciplinary group (IDT) to provide direct supportive services to palliative and terminally ill patients and their families, and to consult with and support other agency personnel. The social worker will provide assessments of psychosocial problems and define goals and interventions to provide excellent end of life care.
The Hospice/Palliative Social Worker:

Is a member of an interdisciplinary team (IDT) and provides social services in accordance with interdisciplinary plan of treatment

Participates in the preparation, evaluation, and execution of the interdisciplinary plan of care to assure that the highest quality of care is given to those served

Recognizes and utilizes community and family resources to assist the patient and family with palliative care and end of life care needs

Develops the plan of care with the IDT to deal with personal, financial, and environmental difficulties that may interfere with care

Assists with funeral planning

Keeps abreast of social work trends and knowledge

Participates in education both internally and externally to promote professional growth and understanding of hospice/palliative

Maintains continuity of patient care by liaising with other health care professionals

Attends IDT conferences and care plan meetings

Arranges for extended care placement or respite placement when necessary
Requirements:
QUALIFIED CANDIDATES WILL POSSESS:

Master’s Degree from an accredited School of Social Work Required

LCSW or LSW License Preferred
-
Two years minimum experience as a medical social worker.

Knowledge of available community services/resources.

Diploma/degree from an accredited school.
-
Must hold and maintain a valid Indiana driver’s license and automobile insurance.

TB status must be current.

Must maintain current CPR certification.
