COMPANY SUMMARY
The Center for Comprehensive Health Practice (CCHP) is a multi-disciplinary community health center and substance abuse program located in East Harlem New York. Accredited by CARF (the Commission on Accreditation of Rehabilitation Facilities) and supported by OASAS (the Office of Alcoholism and Substance Abuse Services). CCHP serves the most vulnerable communities in New York City, primarily East Harlem and the South Bronx but patients come from all five boroughs of New York City.
We are currently seeking a Re-entry Recovery Specialist to assist in one of our programs and lend value to the CCHP team.
JOB DESCRIPTION
The Recovery Specialist's role is to support others in recovery from a substance use disorder,
The ideal candidate will use their personal experience with addiction recovery and reentry to provide
guidance, support, and mentorship to individuals currently navigating similar challenges. This role is
crucial in helping clients build resilience, develop coping strategies, and foster a sense of hope and
empowerment.
The Recovery Specialist will serve as a role model, mentor, advocate and monitor to recovering individuals
in order to prevent relapse and promote log-term recovery
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Actively participate in weekly supervision with Program Manager
- Attend weekly case conferences in which our counselors, physicians, and psychiatrists meet to discuss the trajectory of patients’ treatment plans.
- Partner and network with outside resources to help patients navigate through obstacles encountered during recovery.
- Assist clients in accessing community resources, including housing, employment, education, and healthcare services.
- Provide person centered and eligibility assessments, service plans, authorization requests and case notes.
- Provide counseling services including coping skills, relapse prevention strategies, wellness management and social skills to assist clients to become more independent and achieve their goals.
- Communicate/collaborate with treatment providers to coordinate care and support patients’ recovery plans.
- Occasionally may need to meet with clients in their home to provide rehabilitation services and escort them to appointments and other services.
- Other duties as assigned by supervisor.
- Maintain accurate and timely documentation of patient interactions, progress, and any incidents, ensuring confidentiality and compliance with organizational policies.
EDUCATION AND EXPERIENCE
- Personal experience with addiction recovery and lived experience in the criminal justice system
- Strong understanding of the challenges associated with addiction and reentry, and the ability to
- relate to patients on a personal level.
- Must have high school diploma or equivalency and CPRA or CRPA-T
- • Minimum of 2 years recovery specialist/peer advocate experience
- • Bilingual in Spanish and English preferred

Pay: $50,000.00 - $52,000.00 per year
