PURPOSE: The Concierge Care Assistant Facility to Home Based Care is a unique position that allows you to work in facilities and in the homes of clients. Working in facilities allows you to grow professionally and gain experience as a caregiver with a variety of patients to learn from. While working in the home provides 1:1 holistic care, patient centered care to clients. You may work solely in the home with clients once transitioned into Concierge Care. This position allows for stable hours when home-based, concierge clients are unavailable. Positions available in York County and Cumberland County, Maine.
As a member of the agency team, the Concierge Care Assistant (PSS) provides in home care to clients of the agency and contracted facilities. Concierge Care Assistant are responsible for assisting with activities of daily living (ADL) for clients of various ages according to the plan of care and reporting changes in the living or health conditions of their client.
EDUCATION, LICENSE, AND/OR CERTIFICATIONS REQUIRED:
- Must have CNA license or be PSS or DSP certified
- Must have 1 year of experience as a CNA/PSS/DSP in home care.
- Must have a high school diploma or equivalent
- Must complete yearly required trainings according to company policy.
- Pass pre-employment screening to include criminal history check, DHHS background checks, driving record check, and sex offender registry check; pass periodic re-screening after hire
- Valid driver’s license
- Proof of insurance and registration
- Vehicle maintained in safe drivable condition
DUTIES AND RESPONSIBILITIES:
- Assist client with personal care activities, including but not limited to: bathing, mouth/skin/and hair care.
- Observe Universal Precautions
- Provide exceptional care services as outlined in the service plan and as requested including but not limited to:
- Assist client to the bathroom, commode or with the bedpan.
- Assist client with transfers and ambulation. (Use of gait belt when appropriate)
- Assist client with laundry activities including: washing, drying, folding, putting cloths away, changing bed linen, mending and ironing clothing.
- Assist client with meals, including but not limited to: cooking, setting up, feeding, storing meals, washing and putting away dishes.
- Remind client to take medications.
- Assist client with shopping, including but not limited to: grocery shopping and personal shopping.
- Assist client with transportation to medical appointments, hair dresser, shopping, etc.

- Home Helping/Light Housekeeping
- Technology Assistance
- Document each visit and include client signature, your signature, title (PSS), and date.
- Responsible for occasional on-call services.
- Submit completed and accurate electronic charts as directed.
- Attend mandated training meetings as determined by management.
- Ability to drive to client(s) locations within sixty (60) miles of your home residence
- Ongoing Infection Control education and training
- Participate in QAPI activities as needed

Pay: $19.00 - $23.00 per hour
