We are working with a world-renowned hospital system to expand an innovative new Hospital at Home program. We are hiring highly skilled Registered Nurses (RNs) with Intensive Care Unit (ICU) and/or Emergency Room (ER) experience to support this exciting initiative! As an integral member of our healthcare team, the RN will play a crucial role in delivering exceptional patient care and promoting well-being in the comfort of patients’ homes.
Responsibilities:
- Provide compassionate and comprehensive nursing care to patients in their homes, ensuring the highest standards of clinical excellence and patient safety.
- Conduct thorough assessments, develop individualized care plans, and implement evidence-based practices to address patients’ unique healthcare needs.
- Administer medications, treatments, and procedures with precision, adhering to prescribed protocols and guidelines.
- Monitor patients’ condition closely. Recognizing any changes or complications and taking appropriate action to prevent deterioration.
- Collaborate with physicians, therapists, and other healthcare professionals to establish coordinated care strategies, optimizing patient outcomes.
- Educate patients and their families on disease management, self-care techniques, and medication regimens, fostering a sense of empowerment and active involvement in the healing process.
- Utilize advanced technology and telemedicine tools to facilitate remote monitoring and consultations, ensuring seamless communication with the healthcare team.
- Maintain accurate and up-to-date patient records, documentation, and progress notes, in compliance with regulatory standards and company policies.
- Participate in ongoing training and professional development opportunities to enhance clinical knowledge and stay informed about emerging healthcare trends and best practices.
- Adhere to ethical and legal principles, maintaining confidentiality and respecting patients’ rights throughout the care journey.
Requirements:
- Valid and unrestricted Registered Nurse (RN) license in the state of New Jersey or Nurse Licensure Compact (NLC).
- Bachelor’s degree in nursing (BSN) preferred; associate degree in nursing (ADN) accepted with relevant experience.
- Minimum of 2 years of clinical experience in a fast-paced ICU or ER setting, showcasing a strong clinical acumen (infusions, wound care, etc.) and critical thinking skills. Ultrasound guided IV skills a major plus.
- Comfortable utilizing technology to determine scheduling, providing care, and reporting patient results.
- Willingness to travel locally (Middlesex and Monmouth Counties) to care for patients (we provide the vehicles and gas).
