Social Services:
Duties and Responsibilities:
· Provide insight and assistance as part of the Admission team.
· Interviews all post admission residents and/or families for pertinent social, financial and historical background. Integrates all information into individualized resident care plans and assessment protocols.
· Maintain medical records according to regulatory standards and as needed.
· Work as a member of the interdisciplinary team in providing for the most accurate assessments, care plans and to resolve issues that occur unexpectedly.
· Attend or direct any committees, meetings, or groups as may be assigned
· Maintain knowledge of community resources in dealing with discharge planning, financial, or other assistance.
· Develop strong relationships with various services agencies and geriatric advocate services for optimum resident care and discharge planning.
· Become a strong resident advocate and promote a positive image of the facility for the residents and families.
· Provide in service education in cooperation with Staff Development Coordinator.
· Conduct resident counseling groups. Individual counseling and/or family counseling groups as needed within the scope of resident care and adjustment.
· Represent department in the absence of the director
· Provide data relating to resident incidents as a means to improve systems through the QA Program.
· Assist residents with issues and requests that may fall out of the medical realm of care and deal with adjustment to the facility and quality of life issues.
· May represent the facility at off site meeting and affairs dealing with resident care, marketing, and or promotion.
· Maintain communication with the director.
· In an emergency may be required to perform duties to maintain reasonable function of the facility and safety to the residents.
· Represent residents in legal matters as advocate for safety and security.
Job Type: Part-time
Pay: From $38.00 per hour
Expected hours: 20.0 per week
