Job Description: Director of Intake – Home Care
**Position Title:** Director of Intake

**Job Summary:**
The Director of Intake is responsible for overseeing the intake process for new clients in a home care setting. This role requires a combination of managerial skills, industry knowledge, and experience in care coordination and client management. The Director of Intake will ensure the efficient and effective processing of client admissions, maintain compliance with home care regulations, and work closely with clients, families, healthcare providers, and internal staff to deliver high-quality care.
Key Responsibilities:
1. Leadership and Management:
- Lead and manage the intake team to ensure high performance and professional development.
- Develop and implement policies and procedures for the intake process to enhance efficiency and effectiveness.
2. Client Intake and Coordination:
- Oversee the client intake process, including assessments, authorizations, and documentation.
- Ensure timely and accurate processing of client admissions in compliance with home care regulations and best practices.
3. Regulatory and Compliance:
- Maintain a thorough understanding of home care regulations and reimbursement systems.
- Ensure compliance with all relevant regulations, policies, and procedures.
4. Communication and Collaboration:
- Communicate effectively with clients, families, healthcare providers, and internal staff to coordinate care and address concerns.
- Foster positive relationships with external partners and stakeholders.
5. Data Analysis and Decision Making:
- Analyze data and interpret trends to make informed decisions that optimize the intake process and client outcomes.
- Monitor key performance indicators (KPIs) and implement improvements as needed.
6. Technology and Documentation:
- Utilize electronic health record (EHR) systems and other software tools to manage client information and track intake processes.
- Ensure accurate and timely documentation of all client interactions and care plans.
Qualifications:
- Bachelor’s or Master’s degree in Social Work, Healthcare Administration, or a related field, or relevant experience.
- Previous experience in a leadership or managerial role within a home care agency or healthcare setting.
- Strong knowledge of home care regulations, reimbursement systems, and best practices in care coordination.
- Excellent communication skills to interact effectively with clients, families, healthcare providers, and internal staff.
- Knowledge of authorizations and insurance processes to ensure timely and accurate approvals for care services.
- Ability to analyze data, interpret trends, and make informed decisions to optimize the intake process and client outcomes.
- Proficiency in using electronic health record (EHR) systems and other software tools for managing client information and tracking intake processes.
Compensation:
- Salary range: $90,000 to $120,000 per year.
