Position Summary:
It is responsibility of the Registered Nurse to assess, plan and coordinate the home care services of patients assigned to him/her. Utilizing physician’s orders and professional skills, the Registered Nurse develops and implements a home health plan that meets each patient’s specific needs and is compliant with GA State Department of Health regulations and Agency Policies & procedures.
Responsibilities:
1. Performs admission assessment initially and within 24 hours of referral or patient’s return from the hospital
2. Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
3. Develops a written plan of care for each patient assigned.
4. Assesses and reassesses patients for pain. Utilizes appropriate pain management.
5. Performs patient care responsibilities considering needs specific to the standard of care for patient’s age.
6. Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical position.
7. Follows the five (5) medication rights and reduces the potential for medication errors.
8. Provides skilled nursing care
9. Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
10. Instructs patient, family members and other health workers to perform or assist in the performance of selected treatment procedures.
11. Supervises the CNA or personal care assistants as per regulations.
12. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health errors.

Experience:
- Nursing: 2 years (Preferred)
