We are seeking a highly motivated Health Coach to join our team. Our Health Coaches manage the care of our elderly and disabled clients within the Home & Community based setting.
Reporting to the Manager, Director of Nursing, the Health Coach works to promote the Structured Family Caregiving program, while establishing relationships with local Care Coordinators, Area Agencies on Aging and other organizations/partners. Additionally, the Health Coaches visits families at home, conducts family-centered assessments, and identifies the support a caregiver needs to help provide care at home.
Overview:
- As a Health Coach with Atlanta Structured Family Care you are responsible for providing guidance and support to family caregivers on the CCSP/SOURCE/EDWP Structured Family Caregiver program (SFC).
- The Health Coach, in collaboration with the Registered Nurse (RN), provides an assessment of the needs of the SFC caregiver and assist with creating the coaching plan.
- Health Coaches are responsible for monitoring and providing ongoing training, and support to family caregivers and the client.
- Health Coaches build collaborative partnerships with local community organizations to promote the benefits of Structured Family Caregiving and to help identify and engage caregivers and families that need support
Responsibilities:
- Assist the client and the Primary Family Caregiver with determining eligibility for assistance and resources to complement the SFC program.
- Assess clients & their families psychosocial, environmental, and financial needs.
- Represent Atlanta Structured Family Services at provider, advocacy and partnership meetings within the community
- Provides ongoing health coaching and training to the Primary Family Caregiver to ensure all client goals are met.
- Maintains effective communication with the Primary Family Caregiver, RN, and Medicaid Waiver Case Manager.
- Monitors the Family Caregivers daily progress notes and tasks performed via Secured Application
- Conducts monthly contact/Supervisory visits with the client and the Primary Family Caregiver.
- Schedules case conferences with the client, the Primary Family Caregiver, the RN, and the Care Coordinators semi-annually.
- Sends summary reports to the Care Coordinators monthly, quarterly, and as needed.
- Assist caregivers in essential documents necessary to enable program participation; support caregiver to set-up required mobile communication app, if needed
- Perform other duties as assigned
Qualifications:
- Must have 2 years experience in home health or with Medicaid waiver programs or similar work is required, preferably working with elders or people with disabilities.
- Must have excellent communication, computer, and organizational skills.
- Someone who is dedicated to quality work
- Ability to work independently essential.
- Possesses knowledge of community resources related to medical issues, housing issues, and financial and social issues for dealing with clients' care.
- Ability to be compassionate, understanding
