Xincon Home Health care Services Inc. is seeking a dedicated and passionate health-related field health worker to join our team as a Service Coordinator in the NHTD (Nursing Home Transition and Diversion) and TBI (Traumatic Brain Injury) program serves as a central point of contact for individuals enrolled in these programs. They are responsible for the coordination and management of all services for the individuals they serve, ensuring that their needs are met and their rights are respected.
Responsibilities:
1. Develop and manage comprehensive service plans for individuals enrolled in the NHTD and TBI program.
2. Coordinate with healthcare professionals, social workers, and families to assess the needs of program participants.
3. Monitor the services provided to ensure they meet the agreed-upon plans and the participants’ needs.
4. Act as a liaison between participants, service providers, and other stakeholders.
5. Conduct regular meetings with service providers to evaluate the effectiveness of the services rendered.
6. Ensure full compliance with all state and federal regulations related to NHTD and TBI programs.
7. Assist program participants in identifying and accessing needed services and resources.
8. Document and maintain accurate records of services provided and participants’ progress.
9. Provide crisis intervention as needed and make referrals to appropriate resources.
10. Participate in ongoing training and professional development activities.
Qualifications:
1. Bachelor's degree in Social Work, Psychology, Nursing, or a related field.
2. At least 2 years of experience working with individuals with traumatic brain injury or in a nursing home setting.
3. Knowledge of state and federal regulations related to NHTD and TBI programs.
4. Excellent interpersonal and communication skills.
5. Strong organizational and coordination skills.
6. Ability to work independently and as part of a team.
7. Proficiency in electronic documentation systems.
8. Current driver's license and reliable transportation.
Competitive Pay Range: $110 per case per month
Job Type: Part-time
Pay: $35.00 - $55.00 per hour
