A liaison to the medical nursing unit who provides communication between all disciplines as well as patients, families, and visitors. Health Unit Coordinators are responsible for accurate order entry and management of patient medical chart.

JOB RESPONSIBILITIES
Essential
Collaborates and communicates with the nursing staff to maintain accurate records during the patient's hospitalization.
Accurately enters patient data into computer on all admissions, discharges, and transfers. Enters all computer orders within two hours and processes STAT orders immediately. Faxes orders to KDQue.
Keeps lead nurse informed of admits, discharges, and transfers by frequent communication.
Prepares and maintains patient's charts. Reviews charts at the end of every shift to ensure adequate supply of forms are filed in the chart and that the chart is maintained in proper order. Ensures that after discharge the patient's chart is completely assembled for Medical Records. Prepares chart in proper order for patients transferred to another unit.
Transcribes orders as directed by unit procedures, accurately and legibly in the red book.
Notifies Case Management (when on unit), when orders are written for discharge or discharge planning i.e.: placement, Durable medical equipment, home health, acute transfers ETC.
Obtains Medical Records for physicians from other facilities.
Arranges follow up appointment with Primary Care Physicians (PCP). Refers to Case management if they do not have a PCP.
Files all Lab results appropriately on chart.
Files all Elmer orders and progress notes in chart.
Copies chart and all materials as directed and distributes to appropriate persons.
Is an active participant in achieving quality patient care.
Refers complaints and problems to the Nurse Manager, Assistant Nurse Manager or Team Leader.
Maintains confidentiality with all patient care issues.
Answers patient or family questions or refers them to the appropriate personnel.
Answers phone within four rings.

ORDER PROCESSING DUTIES:
Accurately enters physician orders into computer. Maintains files by unit of orders for reference as needed. Communicates effectively with different departments and people. Utilizes appropriate resources for clarification of orders. Maintains Order Processing Center equipment and supplies. Keeps Order Processing Center a clean and tidy professional workspace. Orients students, new unit staff, and student volunteers to the unit's clerical function. Mentors new hires on computer usage appropriate to their position/unit. Follows all departmental downtime procedures. Able to perform all regular duties when the hospital computer system is not available. Organizes tasks and work time efficiently. Time is used for patient/hospital benefit. Adapts to changes in workload. Manages time to avoid overtime. Breaks and lunches are taken at points that workload will not be affected and return to work in timely manner. No valid complaints that staff is unable to locate a team member at any given time. Corrects errors or omissions in order entry as soon as notified. Copies all materials as directed and distributes them to appropriate persons.
Additional
Assists nursing staff in achieving quality patient care by transporting specimens, observation, and retrieving items needed for patient care. Is certified to pick up blood from the Lab.
Refers complaints and problems to his/her supervisor as appropriate. Holds self and other team members accountable to maintain standards.
Reads and complies with current hospital/patient care procedures and policies as applicable.
