Job Summary:
The Housing Case Manager is responsible for securing and placing survivors in transitional housing by developing partnerships with landlords and creating subsidized rental opportunities. Acting as the primary contact, they coordinate intake assessments, develop service plans, and oversee the delivery of employment skills training, education, economic assistance, therapeutic counseling, children’s services, and legal advocacy. Reporting to the Associate Director of Housing, this role collaborates closely with the Chief Crisis Response Officer and all housing programs.

Monday – Friday
9AM – 5PM
Key Responsibilities:
- Educate landlords on safety policies to ensure client families' well-being and secure stable housing.
- Manage the application process, updates (e.g., income, household size), and recertifications for clients.
- Conduct outreach to community-based organizations, housing resources, and landlord associations to identify and develop housing opportunities, building strong relationships.
- Coordinate intake assessments, eligibility evaluations, and use prescribed assessment tools to determine client needs and services.
- Ensure accurate, timely documentation in client records, complying with agency policies and funding requirements.
- Attend semi-annual meetings coordinated by the Authority.
- Collaborate with A Safe Place (ASP) staff to refer clients to additional services within and outside ASP.
- Coordinate with organizations referring or providing services to ASP’s clients.
- Work with ASP staff to initiate service plans, conduct daily meetings with clients for the first 90 days, and ensure ongoing support.
- Recruit and cultivate relationships with community and corporate partners, providing necessary training.
- Assist in receiving and storing donations.
- Actively participate in staff meetings and demonstrate teamwork within the organization.
- Engage in relevant training and education as required.
- Perform additional assigned duties as needed.

- Excellent written and oral communication skills: Proven ability in record documentation, creating group materials, and delivering clear and concise information during tenant training and education.
- Strong interpersonal skills: Demonstrating genuine concern and respect for clients and colleagues.

Qualifications:
- Bachelor’s degree in social work, counseling, education, or a closely related field.
Preferred Qualifications:
- Bilingual in Spanish preferred.
- Prior experience in a non-profit organization.
- Familiarity with the Sanctuary Model therapeutic approach, modality, and application.
- Relevant certifications in social work or counseling.
Preferred Certifications:
- IMCAN / IMCAT certification (training available).
- 40-Hour Domestic Violence Certificate Illinois (training available).
- First Aid, CPR, and AED Certification/ Card (training available).
