Summary
The Case Manager/Office Manager (CM/OM) at Altura Health plays a pivotal role in supporting and mentoring clients from admission through discharge. This role is multifaceted, focusing on instruction, care coordination, support, family communication, and the smooth operation of office tasks. The CM/OM facilitates clients' transition through treatment phases, ensuring safe and effective reintegration into the community.
Essential Functions
- Case Management Duties:
- Conduct initial and ongoing comprehensive assessments of clients' mental, physical, and psychosocial needs.
- Develop and implement individualized management plans in collaboration with clients and their families.
- Monitor and evaluate the effectiveness of care plans in meeting established health and wellness goals.
- Facilitate group and individual sessions using current methodologies, documenting all client interactions comprehensively.
- Office Management Responsibilities:
- Maintain office organization, manage documentation, and ensure compliance with healthcare regulations.
- Coordinate client appointments and program schedules, ensuring adherence to treatment protocols.
- Oversee the reception area, ensuring a welcoming environment for clients and their families.
Qualifications
- Education: Bachelor’s degree in Social Work, Psychology, or a related field preferred.
- Experience: At least one year of experience in a case management role with strong professional references.
- Certifications:
- Current First Aid and CPR certification.
- Valid driver’s license with a clean driving record.
- Current TB test clearance.
- Skills:
- Strong interpersonal and communication skills, both verbal and written.
- High proficiency in Google Suite, Electronic Health Records (EHR), and basic computer skills.
- Ability to manage stress and adapt in a fast-paced, dynamic environment.
Competencies
- Detailed knowledge and application of federal and state regulations (e.g., FMLA, SSI) within treatment planning and documentation.
- Expertise in biopsychosocial assessments, dual diagnosis treatment, and non-pharmacological symptom management.
- Strong organizational skills with the ability to prioritize tasks effectively and meet deadlines.
Physical Demands
- Regular requirement to talk and hear; frequent standing, walking, and bending.
