Position: PACE Home Care Nurse
Schedule: Monday-Friday, 8am-5pm
Pay: $25 to $36 per hour
JOB SUMMARY:
The Home Care Nurse provides skilled nursing care to PACE program participants within their primary residence. The nurse will work with other IDT staff in the provision of medical and nursing care, ensuring the highest quality of care in accordance with PACE policy and applicable regulations.
ESSENTIAL DUTIES:
- Makes visits to participant homes to assess home care needs.
- Performs initial assessment, annual reassessment, and change of condition assessments of participants in their home environment and coordinates plan of care with appropriate resources.
- When appropriate, provide treatments and health education for participants in their home.
- Ensures compliance with competency requirements.
- Maintains the participant’s medical record with thorough documentation.
- Communicate participant’s changes and progress to appropriate staff.
EDUCATION AND QUALIFICATIONS:
- Must possess a current LPN license in the state of Colorado
- 2+ years working as an LPN
- Experience working with the frail or elderly population, preferred
- Must successfully perform position-specific competencies prior to assuming participant care as an LPN
- Must successfully pass agency required background checks and drug screening
