Around 120 residents requiring various levels of support live in residences on 160 beautifully landscaped acres in a campus-like setting. Residents live an enriched life as they live, learn , work, worship and play and are supported by trained professionals. Job Title: QIDP/Case Manager
Department: Residential Services
Reports To: Director of Residential Services
Position Classification : Salaried andNon-exempt
Scope and/or Purpose: The QIDP/Case Mgr. participates in the ongoing development and management of services to the residents. As an employee of the Residential Division, the incumbent is expected to work in a manner that promotes teamwork, cost efficiency, and safety, and understands and complies with The Baddour Center Code of Ethics. The Baddour Center uses positive behavior supports to shape resident behavior.
Essential Job Duties and Responsibilities:
· Manages and coordinates all services for residents on a caseload, assists the resident to choose activities and classes according to type and schedule offered.
· Serves as informed representative in resident support meetings
-Assists residents to purchase items needed / make shopping lists
-Maintain communication with families regarding all aspects of life at Baddour, respond timely to family requests
-Assist resident to seek medical services if/when needed
· Develops services and manages growth to meet the needs of residents
· Produces a monthly report
· Writes notes and contact recordings in Clinician’s Desktop program,
Other Functions
· Assists in ensuring that residents on work lists are present each day they are scheduled
· Actively participate in meetings
· Leads person centered planning meeting with each resident
Minimum Qualifications:
· 24 years of age
·Master’s degree in health services field, social work, or related area with 2 years’ experience managing services for persons with intellectual disabilities or autism
· 3 years of management experience
· Valid driver license for state of residency, ability to drive 15 passenger van

Ability to read and communicate effectively both orally and in writing
Must be able to audibly converse with residents, all staff, residents’ families, and other professionals providing services, both in person and on the phone
Must be able to apply case management skills to ensure quality programming for individual residents
Ability to interpret assessment results, develop a course of action, and communicate the course of action to the family and the direct care staff
Ability to determine individual needs and coordinate and secure services
Must understand the importance of and maintain a high level of confidentiality
Must be able to function within a team environment
Ability to lift, push, pull and carry up to 30 pounds
Ability to stand, walk, and sit in one place - occasionally may need to squat and/or kneel
Ratio of walking-standing/sitting; 50%/50%
Manual dexterity is necessary for paper/computer work
Requires functional vision and hearing
40% of work time is spent in office setting
Requires critical thinking and good decision-making skills
