Skilled Nursing Facility looking for an MDS Coordinator
Responsibilities:
- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for all patients in a healthcare facility.
- Ensure accurate and timely completion of MDS assessments in compliance with federal and state regulations.
- Collaborate with interdisciplinary teams to gather necessary information for MDS assessments.
- Review medical records, conduct interviews, and observe patients to gather data for MDS assessments.
- Develop and implement individualized care plans based on MDS assessment findings.
- Monitor and update care plans as needed to ensure quality care delivery.
- Provide education and training to staff regarding MDS assessment process and documentation requirements.
- Collaborate with discharge planning teams to ensure smooth transitions of care for patients.
- Stay updated on current regulations, standards, and best practices related to MDS assessments.
Experience:
- Associate's degree in Nursing or related field required.
- Minimum of 2 years of experience in health information management or a related role.
- Knowledge of MDS assessment process and regulations.
- Familiarity with electronic health record systems such as Athenahealth, eClinicalWorks, or similar platforms.
- Experience working in a nursing home or long-term care setting preferred.
- Strong understanding of pediatric care and hospice care principles is a plus.
