Our Home Health Patient Care Coordinator will
- Provide the best patient care around
- Coordinate with clinical staff to ensure efficient admission of new referrals
- Ensure effective and timely coordinator of client home health services through the completion of required documentation and computer data entry for new referrals, as well as timely transfer of pertinent medical data to client’s physician and clinicians.
- Process insurance authorizations
- Answer phone calls using profession etiquette while maintaining patient confidentiality
- Love working in a fast paced environment with a supportive team
- Be great communicator with customers, patients and team members

Qualifications:
- Previous experience in patient intake, medical office, or other healthcare roles
- Experience processing insurance authorization preffered
- Self starter with ability to organize, multitask, and prioritize workload independently in a fast-paced environment
- People oriented person who communicates professionally in person, on the phone, and through email
- Knowledge of medical terminology, anatomy, and physiology ideal, but willing to train
- Proficient use of Microsoft Office, including Excel and Word
- Available to work in our Appleton office
