Position: Part Time to Full Time (20 – 40+ hours/week) Life Care Manager

We have been providing caregiving services for over 40 years in Los Angeles and have enabled thousands of older adults to age in the manner of their choice, most often at home with quality dignified care. Since late 2019 we have provided Private Geriatric Life Care Management services as well.

We are looking for an experienced Life Care Manager, one who is an expert and has solid experience in working with older adults. Our clients are generally clients with resources, who expect “top of the line” service.

Tasks involved:
· Uses professional care management skills to independently assess and re-assess client status, including physical and mental health, social resources, environmental safety, economic resources, and functional ability.
· Drafts, implements and revises the client Plan of Care and Caregiving Plan, to ensure appropriateness, effectiveness, efficiency, and client satisfaction.
· Provides on-going oversight through home visits, and phone calls
· Collaborates with families, guarantors and other service providers to facilitate care coordination and information sharing within HIPAA and other regulatory guidelines.
· Ensures effective communication with clients, families and in-home staff through ongoing home visits, meetings with members of the client committee, telephone contact, email updates, and written documentation.
· Provides supportive counseling to and conflict resolution with clients and family members; serves as an advocate for the well-being of the client.
· Makes referrals to appropriate select community agencies such as crisis intervention, legal advocacy, and counseling agencies for medical, mental, physical, financial, legal, and social needs.
· Screening initial calls of those looking for service (Inquiries)
· Communicates elevated care issues with appropriate internal professionals, CEO, COO, etc…
· Extends client expected length of stay (ELOS) through the delivery of exceptional care management services and consistently exceeding client expectations.
· Interviews, matches, trains, and supervises in-home staff to ensure client needs are met.
· Maintains productivity targets based on company guidelines.
· Anticipates client/caregiver issues in order to maintain quality care within client home, and investigate potential risk management issues.
· Performs all responsibilities in accordance with professional codes of ethics, JFS Care policies and procedures, and applicable state and federal regulations.
· Documents and updates activity to ensure compliance with company protocols.
· Collaborates with community organizations to promote awareness and understanding of elder care issues.
· On-Call for your clients
· Other duties as assigned.

A few details about the position:
● This is a part time to full time postion 20 to 50 hours per week (we would consider either)
● Salary Range: Starting at $37.50/hour (based on experience and hours candidate would like to be working)
● This is a field position, with some office time required
● Our clients primarily reside in West Los Angeles, San Fernando Valley, and other LA County locations requiring driving
