The Transition of Care Coordinator is responsible for managing and auditing resident and employee engagement surveys, making updates in software as needed, ensuring resident satisfaction by compelting discharge survey phone calls to recently discharged residents., and compiling data to be reviewed for trends that can be implemented to raise resident and employee satisfaction.
