Your Opportunity
We are seeking a Full Time R.N. for Home Health. Fleet Landing has over 140 healthcare beds including a post-acute unit, home health, long-term care, assisted living, and memory care.
We are seeking an R.N. whose primary responsibility is supervising clients' care on the Home Health caseload. Follows approved medical procedures in all aspects of Resident care.

Key Responsibilities:
- Completes the initial assessment within 24-48 hours of client admission and prepares care plan.
- Responsible for the observation, assessment, nursing diagnosis, counsel, care, and health education of the ill, injured, or infirm and maintaining health prevention of illness of others.
- Explains all procedures, treatments, medications, and client's plan of care to client and family as appropriate.
- Charts progress notes on hard copy or in e-format, reviews client status and care provided.
- Provides progress notes to the client's healthcare provider; reports when the client's condition changes or when there is a deviation from the plan of care or at least every 60 days.
- Submits required documentation on time.
- Directs the activities of clinical care to L.P.N.s, CNAs, and other ancillary staff
- Acts as coordinator of the health care team to maintain a continuum of care with all disciplines.

Qualifications:
Minimum Education Required:
Must be a Registered Nurse. Must hold a valid license for the State of Florida—current valid C.P.R. certification required.
Experience:
One to three (1-3) years of nursing experience and home care experience is preferred. A minimum of one (1) year of administrative and/or supervisory experience responsible for overseeing nursing and home health aide service delivery is also required. Working knowledge of home care regulations is also preferred. Demonstrated management and leadership expertise is also preferred.
