Job Overview: VIRGINIA AND/ OR MARYLAND LICENSE REQUIRED
We are seeking a dedicated self starting and motivated Nurse Practitioner to join our team for our home based program providing interim care for patients transitioning from inpatient to home setting.
Duties:
- Conduct assessments, diagnose illnesses, and develop treatment plans for patients with acute care needs
- Provide specialized care for individuals with developmental disabilities, focusing on pain management and acute pain management
- Adhere to high standards of patient care and follow protocols set by the facility
- Utilize Cerner electronic health records system to document patient information accurately
- Monitor vital signs, administer medications, and coordinate with healthcare team members to ensure comprehensive care
Experience:
- Previous experience in acute care settings, particularly in a Level II Trauma Center, is highly desirable
- Strong knowledge of pain management techniques and acute pain management protocols
- Familiarity with Tebra or similar electronic health records systems is preferred
- Excellent communication skills and the ability to educate patients on their conditions and treatment plans

Pay: $68.49 - $74.70 per hour
